Understanding the Federal Program Itself

The government itself administers Original Medicare, which comes in two pieces. One (A) handles hospitals; the other (B) handles doctors and outpatient care. Before you think about Advantage plans, supplements or drug coverage, it helps to understand this foundation, because everything else is built on top of it.

This guide walks through the basics in plain words, with the 2026 dollar figures. If you would rather just talk it over, a free call takes about half an hour.

Part A: Hospital Insurance

Inpatient hospital care, short rehab stays in a skilled nursing facility following a qualifying admission, hospice and certain home health visits fall under Part A. Ten years of Medicare payroll taxes, by you or a spouse, usually make it premium-free.

The 2026 deductible is $1,736 per benefit period. Each period opens on admission and closes once 60 straight days pass with no inpatient or skilled nursing care, which means a hard year could bring two or more deductibles.

Part B: Medical Insurance

Part B covers doctor visits, outpatient surgery, lab work, X-rays and scans, durable equipment such as walkers and oxygen, many preventive screenings and ambulance rides. Most enrollees pay $202.90 monthly this year, usually withheld from Social Security. Higher-income households pay an extra amount.

After a $283 yearly deductible, you usually owe 20 percent of the Medicare-approved amount for most services. Preventive care like an annual wellness visit, flu shots and many cancer screenings typically costs nothing when your provider accepts assignment.

What Original Medicare Leaves Out

  • Cleanings, fillings and dentures
  • Routine vision checks and, except after cataract surgery, eyewear
  • Hearing aids
  • Pharmacy prescriptions, which need Part D
  • Extended custodial stays in nursing homes
  • Any limit on what you spend in a year

Those gaps are why most people add something: a Medigap policy plus Part D, or a Medicare Advantage plan that bundles benefits. Qualifying for NJ FamilyCare as well? A D-SNP may fit.

Signing Up

Many people are enrolled automatically if they already receive Social Security retirement benefits before 65; the red, white and blue Medicare card arrives in the mail. Everyone else applies through Social Security online, by phone or at an office.

Your sign-up window and the penalties for missing it are covered on our turning 65 page.

Talk It Over at No Charge

Irvington Questions About Original Medicare

Can I use Original Medicare at any hospital?

Yes, at any hospital or doctor in the United States that accepts Medicare, with no network or referral rules.

Does Original Medicare work outside the country?

Very rarely. For trips abroad, some Medigap letters and some Advantage plans include emergency coverage.

Do I have to take Part B if I am still working?

Not always. Active-job insurance from a sizable employer, yours or a spouse's, can let you postpone it safely. Check before you decide.

Is a Medicare number needed to talk with you?

No. We can explain your options without it. It comes up only at the moment you choose to sign up.
